Graduate Minor Form

Graduate Minor Form (DocuSign)

A graduate student uses the DocuSign form to request adding a Graduate Minor to your academic record.

Complete the document with all required information: 
•    Your current major advisor or department coordinator must approve the document first. 
•    Enter your Graduate Minor information and advisor or department coordinator. 
•    Please allow 3 to 7 business days to see the requested change on your student record. 
•    Contact your current advisor if you need assistance completing the form. 
•    Students pursuing a Doctoral Minor must complete the Graduate Education DocuSign form in addition to submitting this request
