Graduate Minor Form (DocuSign)
A graduate student uses the DocuSign form to request adding a Graduate Minor to your academic record. Students must work with their current degree/program advisor or department coordinator to confirm that they approve of the minor addition before completing the request in DocuSign.
Complete the document with all required information:
- Sender adds the appropriate DocuSign recipients, current degree/program advisor and the Graduate Minor advisor.
- Click SEND.
- The sender/student is the first recipient and can then work within their DocuSign account or use the emailed document link to add information to the document.
- Please allow 3 to 7 business days to see the requested change on your student record.
- Contact your current advisor if you need assistance completing the form.
- Students pursuing a Doctoral Minor must complete the Graduate Education DocuSign form and the Graduate Minor form if the Graduate Minor is the declared Doctoral Minor.
